Referral FAQs

Who can make a referral?

If you have a friend, family member or a patient whom you think would benefit from one of our programs, call us! Many of our programs are open to referrals from the community while others require that the patient come through a particular process (i.e. from the state hospital). The best way to find out how to get into a SERV program is to call us at 1-800-987-SERV (7378). We can help you navigate your way to us.

Who is eligible for SERV programs?

Different programs have somewhat different eligibility criteria. Please call us for specifics.
